New Population Health Tools in Cerner

 

On October 1st McLaren Physician Partners (MPP) fully integrated Persivia CareTrak into Cerner for primary care providers.    Persivia utilizes HEDIS guidelines and evidence-based practices to identify preventive or chronic condition specific services that may be needed based on the patient’s demographics and medical history.  

 

Within Cerner, Persivia CareTrak allows providers to visualize HEDIS metrics while the patient is in front of the provider.  Population health information includes:

  • CareView Card displays payor required HEDIS metrics (examples: Breast Cancer Screening or Blood Pressure Control)
  • Gaps in Care for the current patient and other population level data (example: list of patients needing a Statin medication for cardiovascular disease)
  • Quality Metric and Clinical Integration Scorecards allowing physicians to track to their performance over time.

 

Utilization of CareTrak generates 15 points on the PCP and Pediatrician Clinical Integration Scorecard.

 

Use of these tools will improve quality performance in value-based care programs and enhances management of population health, thereby improving the quality and health in our communities.
