What Is A Swing Bed Program?

The Swing Bed Program at McLaren Caro Region and McLaren Thumb Region offers transitional care for those patients who no longer require acute medical care but aren’t quite ready to go home. Traditionally, these patients would be transferred to a skilled nursing facility, nursing home, or rehabilitation facility—often outside of the community—to receive the appropriate level of care. For patients who transition into our Swing Bed Program who are currently receiving inpatient care at McLaren, they are able to remain in the same hospital setting where their bed “swings” from an acute care bed—which requires more intensive care—and transitions to a bed requiring less intensive rehabilitation services and skilled nursing care. For qualified patients receiving inpatient care at outside facilities, they are still able to transition into our Swing Bed Program.

Our highly skilled multidisciplinary team provides 24-hour on-site care, individualized treatment plans, as well as physical, occupational, and speech rehabilitation services, all while staying in a familiar environment close to home. If the situation arises, our patients also have 24/7 access to on-site emergency care.

HOW DO YOU QUALIFY FOR A SWING BED PROGRAM?

Our Swing Bed Program offers several features and services. You may qualify for this program if you require:
  • Rehabilitation therapy after surgery, illness, or an injury
  • Rehabilitation after a stroke
  • Certain types of wound care that require evaluation and daily treatment by healthcare professionals
  • Therapy to assist with debilitation or weakness
  • IV Therapy
The Swing Bed Program at McLaren is a federally approved program. To be eligible you must:
  • Rules vary by payor. Patients with Medicare must have had a three-day acute care hospital stay immediately prior to being admitted
  • Be insured by Medicare Part A or other insurance with hospital-based skilled care benefits
  • An ongoing diagnosis or need requiring skilled care services related to the acute inpatient hospital stay that can only be provided in a skilled nursing facility
  • Have a stable medical status
  • Have physician authorization

WHAT IS THE AVERAGE LENGTH OF STAY?

A patient’s length of stay is dependent on their individual needs and progress. As with all our patients in our hospitals, our goal is to get our patients back home or to the next appropriate level of care as soon as possible.

WHAT IS THE COST?

Our experienced team will verify your eligibility and receive prior authorization from your insurance. For those with Medicare, the first 20 days of Swing Bed care are completely covered if benefits are available. Medicare has a daily co-pay for days 21 – 100, and after 100 days, patients are responsible for all costs.
