What is home health care?
Home health care is a range of health care services provided at home to treat an illness or injury. Examples of skilled home care services include wound care for pressure sores or a surgical wound, intravenous or nutrition therapy, monitoring of a serious illness or unstable health status, and rehab therapy services. Home health care can be provided anywhere you call home, including at an assisted living facility or adult foster care home.
Home health care may be right for you after an inpatient hospital stay, if you recently received a joint replacement, or if you have a chronic condition including chronic obstructive pulmonary disease (COPD), congestive heart failure, diabetes, kidney disease or respiratory disease, among others.
Qualifications for home health care
To start home health care, a doctor's orders are required. These orders will usually come from your primary care physician. After receiving your doctor's orders for home health care, we will coordinate the services ordered by your physician.
Most insurance plans, as well as Medicare and Medicaid, cover all or part of home health care. To be eligible for home health care under Medicare Part A (hospital insurance) and/or Part B (medical insurance), you:
- Must be under the care of a doctor, and must be getting services under a plan of care established and reviewed regularly by that doctor
- Must have a need for one or more of the below, as certified by a doctor:
- Intermittent, limited skilled nursing care (other than drawing blood) that requires oversight of a nurse
- Physical therapy, occupational therapy, or continued speech-language pathology services
- Must be homebound, as certified by a doctor
What is homebound?
To be homebound means a patient has trouble leaving home without help because of an illness or injury or leaving the home is not recommended because of the patient's condition, and the patient is normally unable to leave the home because it's a major effort. You may leave home for medical treatment or short, infrequent absences for non-medical reasons, like attending religious services. You can still get home health care if you attend adult day care.
Insurance Coverage
Before you begin home health care services, we will let you know how much your insurance, Medicare or Medicaid will pay. We will also tell you if any services are not covered, and how much you will be responsible for paying out-of-pocket. It is important to ask questions so you understand why your doctor is recommending certain services and whether your insurance will pay for them.
How to get home care
A doctor's orders are needed to start home health care. Once your doctor refers you for home health services and you choose McLaren, we will schedule an appointment with you. At that appointment, we will come to your home to talk to you about your needs, and ask you some questions about your health. We will also talk to your doctor about your care and keep him or her updated on your progress.

- Joint Express at Home: helps patients return home safely after hip and knee surgery
