Frequently Asked Questions

A specific list of medical information must be submitted to be approved for admission. The following information is required and can be obtained from your primary care physician and faxed to the facility:

• History and physical with medical diagnosis
• Current chest x-ray within the last 30 days
• A list of current medications with dosing information
• A mental health screening form
• A residency application to be completed

Lake Orion Nursing and Rehabilitation Center is a smoke free campus. Smoking is strictly prohibited on the premises.

In order to complete the necessary paperwork, the following items are required:

• Medicare/Medicaid card
• Commercial Insurance card
• Driver’s License or state identification card
• Prescription card
• Legal documents such as a Durable Power of Attorney or Guardianship papers

Yes, residents who are unable to feel themselves are assisted by a staff member at every meal.

We discourage residents from keeping money in their rooms or on them at any time. Lake Orion Nursing and Rehabilitation Center strongly encourages residents to participate in the individual trust fund account program that is managed by our business office.

Lake Orion Nursing and Rehabilitation Center offers private and semi-private rooms.

When you express interest in our facility, the discharge planner at the hospital will contact the Admissions department at Lake Orion Nursing and Rehabilitation Center. All of the appropriate medical documentation is then forwarded and reviewed by our Admissions department. Once approved for admission, the admissions manager will contact the hospital to set up a time of arrival.

Visiting hours are recommended 10:00 a.m. until 8:00 p.m. If visiting before or after these hours use the intercom to call second floor who will buzz you in.

Our activities department has a full calendar of events tailored to meet the needs of our residents. Our experienced activities team works closely with residents to ensure that they have the opportunity to participate in the programs that are occurring on a daily basis. From group activities such as Art Therapy, entertainment and bingo, to community outings, or one-on-one visits, there is an activity available to meet the needs and interests of everyone. We also have church services available for those who are interested.

Medicare, Medicaid, Commercial Insurance, Hospice and private funds are accepted.
