Job Shadowing ("Observation") consent and Release Form for McLaren Macomb

The job shadow experience is designed to be a one-time observation only for two (2) hours.

The job shadow experience does not fulfill any clinical or internship requirements; nor does it provide any hands-on experience. Job shadows are typically only available Monday through Friday from 7 a.m. to 5 p.m.

As part of the application process, you will be required to read and agree to the terms of our Confidentiality Agreement and acknowledgment of Responsibilities. Please see attached.

Parents of minors will also be required to read and agree to these terms.
