McLaren Physician Partners Introduces

Homeward Health

In January 2025, BCBSM and Aetna retained Homeward Health to provide supplemental services to members in the rural health settings.MPP is currently working with Homeward, on your behalf, to identify patients who would benefit from these services.

What to expect from Homeward Health:

• Provides Community Health Workers to serve patients in rural communities in their homes
• Works with the patient’s PCP as part of the care team (does not replace primary care)
• Focused on value-based care, preventative care, and treating the whole patient

  Services:

• Coordination of clinical and non-clinical services
• Social determinants of health screening and alignment with community resources such as access to food, transportation, or housing
• Gaps in care closure
• In-home assessments
• Behavioral health resource navigation
• Encourage screenings and AWVs
