Home health care is a range of health care services provided at home to treat an illness or injury. Examples of skilled home care services include wound care for pressure sores or a surgical wound, intravenous or nutrition therapy, monitoring of a serious illness or unstable health status, and rehab therapy services. Home health care can be provided anywhere you call home, including at an assisted living facility or adult foster care home.

Home health care may be right for you after an inpatient hospital stay, if you recently received a joint replacement, or if you have a chronic condition including chronic obstructive pulmonary disease (COPD), congestive heart failure, diabetes, kidney disease or respiratory disease, among others.

Inpatient rehabilitation at McLaren Bay Region, McLaren Flint, McLaren Macomb, McLaren Northern, and McLaren Oakland helps patients achieve their highest level of independence after serious injury, illness, or surgery.

We work together to develop goals that put you on the path to recovery. Each patient receives at least three hours of therapy on five or more days a week. We target strength and balance as well as cognitive and motor skills. The length of stay is individualized by what you need, your progress and your ability to participate. Our goal is to get you back home functioning as well as possible.

Palliative care focuses on improving your quality of life while providing relief from the symptoms of both your disease and treatment (such as pain, depression, fatigue, shortness of breath, or nausea). It is for people living with serious, complex and chronic illnesses such as the following:

The Swing Bed Programs at McLaren offer transitional care for those patients who no longer require acute medical care but aren’t quite ready to go home. Traditionally, these patients would be transferred to a skilled nursing facility, nursing home, or rehabilitation facility—often outside of the community—to receive the appropriate level of care. For patients who transition into our Swing Bed Program who are currently receiving inpatient care at McLaren, they can remain in the same hospital setting where their bed “swings” from an acute care bed—which requires more intensive care—and transitions to a bed requiring less intensive rehabilitation services and skilled nursing care. For qualified patients receiving inpatient care at outside facilities, they are still able to transition into our Swing Bed Program.

Our highly skilled multidisciplinary team provides 24-hour on-site care, individualized treatment plans, as well as physical, occupational, and speech rehabilitation services, all while staying in a familiar environment close to home. If the situation arises, our patients also have 24/7 access to on-site emergency care.
