CONTINUUM OF CARE

Home health care is a range of health care services provided at home to treat an illness or injury. Examples of skilled home care services include wound care for pressure sores or a surgical wound, intravenous or nutrition therapy, monitoring of a serious illness or unstable health status, and rehab therapy services. Home health care can be provided anywhere you call home, including at an assisted living facility or adult foster care home.

Home health care may be right for you after an inpatient hospital stay, if you recently received a joint replacement, or if you have a chronic condition including chronic obstructive pulmonary disease (COPD), congestive heart failure, diabetes, kidney disease or respiratory disease, among others.

Inpatient rehabilitation at McLaren Bay Region, McLaren Flint, McLaren Macomb, McLaren Northern, McLaren Oakland and McLaren Lapeer helps patients achieve their highest level of independence after serious injury, illness, or surgery.

We work together to develop goals that put you on the path to recovery. Each patient receives at least three hours of therapy on five or more days a week. We target strength and balance as well as cognitive and motor skills. The length of stay is individualized by what you need, your progress, and your ability to participate. Our goal is to get you back home functioning as well as possible.
