Newly diagnosed cancer patients find themselves in a new and complex world of terminology, testing, and treatment. Oncology Nurse Navigators at McLaren Northern Michigan assist patients every step of the way, making sure patients have the support and information needed to stay committed to their fight against cancer. Nurse Navigators help patients find the right health care professionals for their needs, help patients make sense of diagnosis and pathology reports, coordinate appointments, and act as a resource for patients and loved ones throughout the treatment journey.
Care Coordination: From Hospital to Home
Care Coordinators at McLaren Northern Michigan not only work to ensure that patients understand their treatment while in the hospital, they also work to ensure that appropriate ongoing care is available for patients after discharge. Care Coordinators are skilled at matching services and information to patient needs. From outpatient rehabilitation at the John and Marnie Demmer Wellness Pavilion, to ongoing home care, private duty nursing, home medical equipment, or telehealth services available through McLaren Home Care, the McLaren Northern Michigan health system provides a full continuum of care appropriate for individual patient needs.
